New Patient Form
YOUR FIRST VISIT
Please arrive about 10 minutes early to allow time to complete a New Patient Form. To make your first visit even easier, you can fill out our New Patient Form online* before your appointment.
We encourage everyone to seek regular dental care and Dr. Bensimon will develop a specific plan for your oral health needs. Patients who are at a greater risk for oral disease may be required to come in more frequently. A check-up allows your dentist to assess your overall oral health and helps in detecting early problems like cavities and gum diseases.
INITIAL EXAMINATION:
Your Dental History
We'll review your dental and medical care history, as well as any concerns you currently have about your oral health.
Complete Oral Exam
We'll perform a complete oral exam, including X-rays to help us assess the overall health of your smile.
Oral Health Plan
We'll prepare recommendations for your total oral health, which will include full oral maintenance, as well as the potential of additional dentistry services that we agree upon.
YOUR FIRST APPOINTMENT
Your first appointment will generally last about an hour and a half.
This covers a full dental examination with digital x-rays if necessary.
A FULL MOUTH SET OF X-RAYS
These, which are occasionally required, are among the most vital tools that dentists use for keeping your teeth and gums healthy.
- Radiographs or x-ray images play an important role in diagnosing problems associated with the structures of the teeth, jaw and the mouth.
- X-rays –may show areas of decay that may not be visible with visual examinations alone. It is used to find cracks, possible bone loss during periodontal (gum) diseases and other abnormalities like cysts or any other changes linked with metabolic and systemic illnesses.
- Radiographs or x-rays are used as well, in conjunction with major treatment such as implants, crowns and bridges.
A COMPREHENSIVE DENTAL EXAM
A comprehensive dental exam entails each and every aspect concerning your oral health.
- Full Dental Exam (teeth)
- Charting of restorations and gum levels
- Head and Neck Cancer Exam (soft tissues of the mouth, throat and the surrounding structures)
- Occlusal Exam (bite analysis)
- TMJ Exam (temporomandibular joint or jaw joints)
- Radiographic Exam (x-rays and other scans)
- Aesthetic Exam (appearance of the teeth and gums
- Documentation of patient concerns
Each person has unique needs.
Our team will go over all treatment plans and options with you to help you to decide the best treatment possible within the confines of your personal insurance and financial position.
